Liquid Personnel are proud to be working with Richmond and Wandsworth Councils to recruit a passionate and experienced Hospital Discharge Mental Health Social Worker to join their dynamic, integrated social work team.
This is a fantastic opportunity to support the safe and timely discharge of residents from hospital settings, working closely with community health and hospital trust partners across Richmond and Wandsworth.
- Competitive salary up to £52,767
- £3,000 retention bonus (paid every two years)

- Access to Research in Practice membership and Institute of Leadership and Management resources
- Work with residents admitted to hospital due to mental health needs, including acute, rehab, forensic, and general wards
- Prevent unnecessary hospital admissions through close collaboration with health colleagues
- Facilitate timely and safe hospital discharges, reducing delays
- Complete Care Act assessments and develop strength‑based support plans
- Empower individuals to have choice and control over their care while ensuring safety and proportionate support
- Conduct mental capacity assessments and facilitate best interest decisions
- Registration as a Social Worker with Social Work England
- At least one year of post‑ASYE experience in a statutory setting
- Experience in case management, risk assessment, and multi‑agency collaboration
- Experience in hospital discharge is desirable, but not essential
