Responsibilities- Provides social work services to patients, their families and/or significant others through counseling, emotional support, assisting with environmental needs, crisis intervention, and group leadership.
- Provide psychosocial assessments of patients and families to identify emotional, social, and community strengths and problems related to their diagnosis, illness, treatment, and life situation.
- Participates as an integral member of an interdisciplinary treatment team led by a physician.
- Formulate, develop, and implement comprehensive treatment plans utilizing appropriate and evidence-based treatments and interventions. Interventions may include crisis intervention and solution-focused brief intervention.
- Screens, educates on diagnosis, and provides therapy and interventions to individuals with various presenting problems including substance abuse, depression, anxiety, thought disorders, trauma, and autism spectrum.
- Provide consultative services to health care team members within scope of care definitions; maintain knowledge of relevant medical/legal issues (e.g., advance directives, child and elder abuse).
- Educate patients and families about adaptation to diagnosis, illness, treatment, and life situation.
- Participate in multi-disciplinary health care teams and represent clinical social work perspective with patients and families.
- May formulate and implement discharge plans starting on admission through length of stay.
- Act as a liaison with community agencies and resources to assist patients and families as needed.
- Facilitate cognitive/psycho educational group therapy at least daily and provide individual Solution Focused Brief therapy and crisis intervention as ordered by doctor or as needed.
- Provide updates to families/support persons and care providers; facilitate family meetings for discharge planning and education as applicable.
- Facilitate referrals to placement options (e.g., personal care home, nursing home, skilled rehab, assisted living, group homes).
- Document assessment, plan, interactions, and interventions per department, hospital, and health system guidelines. Maintain electronic medical records and participate in staff/committee/department meetings.
Education, Certification, and/or Licensure:
- For West Virginia locations:
Master’s Degree in Social Work or related field;
Licensed Graduate Social Worker (LGSW), Licensed Certified Social Worker (LCSW), or Licensed Independent Clinical Social Worker (LICSW) through West Virginia Board of Social Work. - For Ohio locations:
Master’s Degree in Social Work or related field;
Licensed Social Worker (LSW), Licensed Independent Social Worker (LISW), or LISW-S through Ohio Counselor, Social Worker, and Marriage and Family Therapist Board. - For Maryland locations:
Master’s Degree in Social Work or related field; LGSW/LCSW or LISW-C through Maryland Board of Social Work Examiners. - For Pennsylvania locations:
Master’s Degree in Social Work or related field.
Preferred Qualifications
- Certification in Case Management.
Experience
- Two (2) years’ social work experience.
- Completion of annual required education related to specialty accreditation as defined by accreditation standards.
- Prolonged periods of standing or sitting.
- Flexible hours to include weekends and holidays for inpatient setting.
- Excellent verbal and written communication skills.
- Ability to collaborate with health care professionals at all levels to achieve goals and improve quality outcomes.
- Basic computer knowledge and ability to operate standard office software.
Job Description
The selected candidate will primarily work remotely providing services/support to the Weirton and surrounding area and will be asked to obtain a Master’s-level licensure in the state of Ohio within 6 months of hire. Candidates may be required to attend in-person events on occasion; sufficient notice will be provided beforehand.
