Position: Licensed Practical Nurse (LPN) – Home Health
Location: West Saint Paul

Schedule:

Full-time | 64 hours/pay period | Monday to Friday | 8 a.m. to 4:30 p.m.

Pay Range: $24.50 - $34.00/hour (non-exempt), based on experience, qualifications, and location.

As a LPN in Home Health, you’ll be at the heart of our community, delivering compassionate and personalized skilled nursing care to clients in the comfort of their own homes. Whether providing direct clinical care, educating clients and families, or ensuring compliance with state and federal regulations, you’ll play a key role in enhancing the independence, safety, and well‑being of our clients.

What you'd do as a Licensed Practical Nurse (LPN) with Home Health:

Direct Client Care
  • Perform preventative and rehabilitative procedures as directed by the Registered Nurse (RN) to ensure client safety and optimal health. Observe and report changes in the client’s condition, including reactions to medications and treatments.
Educate & Support Clients & Aides
  • Provide teaching, supervision, and counseling to clients and their caregivers, empowering them to manage care needs at home with confidence.
Ensure Accurate Documentation & Billing
  • Maintain timely and precise documentation of services in the agency EMR system, ensuring compliance with regulations and payer source requirements. Complete and document supervisory visits with Home Health Aides (HHAs) per guidelines.
Communicate & Collaborate
  • Serve as a trusted liaison between clients, families, physicians, and interdisciplinary team members, ensuring seamless coordination of care. Maintain clear, compassionate, and professional communication at all times.

This Job Also Requires
  • Current Licensed Practical Nurse (LPN) license in the state of Minnesota.
  • CPR certification (or willingness to obtain within 6 months of hire).
  • Strong clinical skills in home health, client assessment, and medication management.
  • Experience in home health, long‑term care is preferred.
  • Ability to pass state‑mandated background check.
  • Physical capability to perform all essential job functions.
  • Valid driver’s license and access to an insured personal vehicle.
