Position SummaryWe are seeking a compassionate and organized Licensed Social Worker to join our healthcare team. The Social Worker plays a vital role in supporting residents’ psychosocial needs, care planning, discharge planning, and compliance with state and federal regulations.
Key Responsibilities- Complete psychosocial assessments and ongoing evaluations for residents
- Develop, implement, and update individualized care plans
- Assist residents and families with adjustment to illness, placement, and care transitions
- Coordinate discharge planning and community resources
- Provide counseling and crisis intervention as needed
- Advocate for residents’ rights and ensure dignity and quality of life
- Maintain accurate documentation in accordance with state, federal, and facility requirements
- Participate in care conferences and interdisciplinary team meetings
- Ensure compliance with Minnesota Department of Health and CMS regulations
- Licensed Social Worker (LSW) in the State of Minnesota
- Bachelor’s degree in Social Work (BSW)
- Experience in a skilled nursing facility, long-term care, or healthcare setting preferred
- Knowledge of Medicare, Medicaid, and long-term care regulations
- Strong communication, documentation, and organizational skills
- Ability to work collaboratively with residents, families, and care teams
