Southview Acres Healthcare Center is a skilled nursing and long-term care community committed to providing compassionate, resident-centered care. Our interdisciplinary team works collaboratively to support residents and their families through every stage of care.

Position Summary

We are seeking a compassionate and organized Licensed Social Worker to join our healthcare team. The Social Worker plays a vital role in supporting residents’ psychosocial needs, care planning, discharge planning, and compliance with state and federal regulations.

Key Responsibilities
  • Complete psychosocial assessments and ongoing evaluations for residents
  • Develop, implement, and update individualized care plans
  • Assist residents and families with adjustment to illness, placement, and care transitions
  • Coordinate discharge planning and community resources
  • Provide counseling and crisis intervention as needed
  • Advocate for residents’ rights and ensure dignity and quality of life
  • Maintain accurate documentation in accordance with state, federal, and facility requirements
  • Participate in care conferences and interdisciplinary team meetings
  • Ensure compliance with Minnesota Department of Health and CMS regulations
Qualifications
  • Licensed Social Worker (LSW) in the State of Minnesota
  • Bachelor’s degree in Social Work (BSW)
  • Experience in a skilled nursing facility, long-term care, or healthcare setting preferred
  • Knowledge of Medicare, Medicaid, and long-term care regulations
  • Strong communication, documentation, and organizational skills
  • Ability to work collaboratively with residents, families, and care teams
