Nurse Practitioner - Geriatric Medicine

Job Description General Responsibilities

• Conduct thorough medical assessments to inform patient care.
• Design and execute tailored care plans for individual patients.
• Evaluate and treat acute illnesses in various settings.
• Engage actively in discharge planning to ensure continuity of care.
• Perform home visits as needed for patient assessment and support.
• Facilitate seamless coordination of care across the full spectrum of services provided.
• Contribute to the development and review of clinical policies and procedural guidelines.
• Take part in the provider on-call rotation as necessary.

• Board Certified Nurse Practitioner (ANCC or AANP) – [REQUIRED]
• Current, active Nurse Practitioner license in Massachusetts – [REQUIRED]
• DEA Registration to prescribe controlled substances – [REQUIRED]
• Certification in Controlled Substances (CDS) – [REQUIRED]
• BLS Certification – [REQUIRED]
• Relevant experience working with frail or elderly populations – [REQUIRED]
• Willingness and ability to travel within the designated service area – [REQUIRED]

• AANP
• ANCC
• Basic Life Support (BLS)
• Controlled Substance Registration (CSR/CDS)
• Drug Enforcement Administration (DEA) registration

• Valid Nurse Practitioner license in Massachusetts

• Nurse Practitioner certification with prescriptive authority.
