Position Purpose
The day center aide is responsible for providing various care services necessary for attending to the personal needs, support, and health status of participants.
Employment Details- Employment Type: Full time
- Shift: Day Shift
- Participate as a member of the interdisciplinary team in ongoing participant assessment and monitoring, reporting on personal care services provided and self‑care abilities of participants, including changes noted in participants’ condition.
- Provide personal care assistance that maintains participants’ independence in activities of daily living, including toileting, showers, eating, grooming, and assists with physical activity, exercise, and walking.
- Provide nursing care as assigned in a safe manner, consistent with the policies and procedures of the PACE Organization.
- Ensure participant safety by identifying safety hazards and emergency situations and initiating corrective action.
- Offer behavioral supervision or support for participants, such as reality orientation, reminders to follow nursing and medication instructions, diet restrictions, and emotional reassurance.
- Assist with general activities such as conversing with participants, recreation activities, meal and snack serving, and housekeeping.
- Identify and report signs indicating physical, emotional, or behavioral change, including changes in skin integrity, bowel and bladder status, food/fluid intake, or ambulation.
- Ensure compliance with infection control, personal protection equipment, blood‑borne pathogens, hazardous material handling, and fire safety standards.
- Escort participants to medical, dental, optometry, rehabilitation, and other appointments as necessary.
- Assist transportation staff with loading and unloading participants from vehicles and provide support during transport.
- High School Diploma or equivalent.
- Active certified nurse assistant in the state where the PACE Organization is located.
- At least one year of documented experience working with a frail or elderly population, or if less than one year, completion of appropriate training from the PACE organization.
- Experience in a long‑term care facility or a community‑based geriatric program preferred.
- Physical ability to kneel, crouch, reach, lift, sit, walk, bend, and lift. Must be able to push, pull, move, or lift a minimum of 20 pounds.
- Physical mobility, ability to sit for long periods, and coordination including eye‑hand, hand‑foot dexterity.
