Family Practice - Geriatrics Physician

• Conduct history and physical assessments for new participants and periodic reassessments.
• Develop individualized medical treatment plans and provide necessary services in various settings.
• Interview and evaluate applicants and their caregivers to support the Intake and Assessment process.
• Engage with participants to discuss their medical conditions, prognosis, and future care planning.
• Works collaboratively within a shared culture across all divisions of the organization.
• Participate as a member of the interdisciplinary team and advise on diagnoses, function, and treatment options.
• Educate participants and families on health maintenance and chronic conditions.
• Oversee the healthcare and treatment programs developed by the interdisciplinary team.
• Conduct physical examinations, order lab and diagnostic tests, and document results systematically.
• Participate in the provider on-call rotation as required.
• Demonstrate effective physical assessment skills in managing chronic diseases for frail older adults.
• Able to adapt and manage priorities based on participant needs.

• Current and active plenary physician license to practice in Massachusetts. REQUIRED
• Board Certification in Internal Medicine or Family Practice. REQUIRED
• Board Certification in Geriatrics is preferred. PREFERRED
• Experience or additional qualifications in geriatrics is preferred. PREFERRED
• DEA Registration. REQUIRED
• Previous supervisory experience. REQUIRED
• Minimum of one year of documented experience with a frail or elderly population. REQUIRED
• Self-Query NPDB (to be pulled within the month). REQUIRED

• Active Massachusetts physician license.
• Board Certification in Internal Medicine or Family Practice.
• DEA Registration.
