White Oak Medical Center seeks to hire an experienced Nurse Practitioner for our Cardiac department who will embrace our mission to extend God’s care through the ministry of physical, mental, and spiritual healing.

Responsibilities- Identify and enroll patients in the hospital with heart failure into Heart Failure Clinic
- Educate and guide patients on disease prevention and healthy lifestyle habits in the hospital
- Coordinate Patient Care with interdisciplinary team (Physicians, Cardiologists, Case Management)
- Obtain patient history, perform assessment, examination, and obtain labs
- Perform medicine reconciliation
- Educate the patient on chronic disease in Clinic
- Develop plan of care for patient
- Follow‑up calls made to patient daily (Vitals, medicine, symptoms)
- Master's degree or Doctorate degree in Nursing
- Cardiovascular experience preferred
- Licensed as an Advanced Registered Nurse Practitioner in Maryland
- National Board Certification
- ACLS Certification preferred
- Certification as an Acute Care Nurse Practitioner preferred
- Eligible for Maryland Controlled Dangerous Substance license
- DEA license is not required but preferred
- Active American Heart Association Basic Life Support (BLS) certification required

$46.18 - $70.19
