The Hospice Nursing Assistant provides personal care under the supervision of a nurse according to the patient's plan of care. This position will report to the Clinical Director of Hospice.

• Provide personal care and assist with ADL's according to the Plan of Care - POC.
• Provide safe environment for patients.
• Safely ambulate/transfer patient as appropriate.
• Help with ROM, both active and assistant active exercises as directed by the nurse.
• Prepare nutritional snacks as appropriate for the patient.
• Under the direction of the nurse, perform simple, skin care in accordance to the plan of care where appropriate.
• Recognize and promptly report any changes in the mental or physical status of the patient to the nurse.
• Record in clinical record, on appropriate documents, the activities provided and observations of patient at the time of visit.
• Complete all charting, timesheets and mileage logs as defined by the agency policy and procedures.
• Attend and actively contribute during patient care conference.
• Participate in Hospice discipline meetings, team meetings, staff meetings, retreats and in-services as appropriate.
• Participate and/or attend in-services training to obtain 12 education hours per year.
• Participate in the on-call rotation as scheduled
• Assist the Team Leader with other designated activities as necessary (scheduling, supply organization, answering office telephone, etc.)
• Demonstrate positive customer-service responsiveness according to agency standards.
• Flexible in meeting patient/family needs that require variable shifts and weekends.
• Participate in chart review quality improvement activities and other meetings as assigned.
• Participate in communication with volunteer staff.

• Experience in hospice, medical facilities, home care or nursing homes, preferred.
• Hospice/Palliative Care experience preferred.
• Must be state tested certified nursing assistant in good standing and have 12 months experience as a nursing assistant
• Knowledge of community resources.
• Comprehensive knowledge of state/federal regulations and industry standards
• Strong Administration skills as well as computer/software abilities

• Provide basic care-giving education to caregiver/family members, such as turning and positioning, transfer techniques and changing linens
• Complete annual competency requirements.
• Performs other duties as assigned
