This position is based in West Palm Beach, FL, supporting patients in the Palm Beach County area.

As a Home Health LPN, you will provide high-quality, personalized nursing care directly in patients’ homes—supporting independence, recovery, and overall well‑being. Working under the direction of a Registered Nurse, you will play a vital role in delivering compassionate, consistent care.

• Following individualized, goal‑driven plans of care established by the RN
• Collaborating with patients, caregivers, Covenant leadership, and a supportive Concierge clinical team
• Utilizing dictation services to streamline documentation and maximize patient‑facing time
• Delivering hands‑on nursing care, including:
  • Acute and chronic disease management
  • Medication administration and management
  • Post‑surgical rehabilitation support
  • Pain management
  • Wound care

• Unincumbered Florida Licensed Practical Nurse (required)
• Valid driver’s license, auto insurance, reliable transportation (required)
• Home health care experience is preferred, but not required
