$36.00-$46.00 per hour

Beach Breeze Rehab & Care is seeking a skilled and dedicated RN or LPN MDS Coordinator to join our team. This role is essential in ensuring accurate and timely Resident Assessment (MDS) processes in compliance with current regulations, including the implementation of RAPs and Triggers.

• Oversee the development and completion of MDS assessments, ensuring full compliance with regulations.
• Maintain and update MDS and care plan policies and procedures.
• Complete administrative tasks, including medical forms, reports, and evaluations.
• Develop and implement a quality assurance program for resident assessments and care plans.
• Collaborate with the Discharge Planning Coordinator on care plan components of discharge plans.
• Participate in facility surveys and inspections conducted by government agencies.
• Monitor Quality Indicators (QI) and Quality Measures (QM), implementing corrective actions as needed.
• Ensure timely completion and submission of all assessments, reporting any issues to the Administrator.
• Assist in coordinating and scheduling interdisciplinary care plan meetings.
• Work collaboratively with residents’ families and outside care team members.
• Evaluate residents’ abilities and limitations to support appropriate therapy and activities.
• Support therapy services, including assisting occupational therapists with activities.

• Florida RN or LPN License required.
• Minimum of 2 years of nursing home experience preferred.
• Strong collaboration, communication, and teamwork skills.
