Salary: LPN Up to $34 | RN Up to $40

• Conduct comprehensive assessments of potential residents to determine their physical, emotional, and psychosocial needs.
• Collaborate with healthcare providers, families, and residents to gather relevant medical history and information to support the admission process.
• Develop individualized care plans based on assessment findings, ensuring alignment with the resident’s unique needs and preferences.
• Work closely with the interdisciplinary healthcare team to coordinate and implement care plans.
• Effectively communicate with prospective residents and their families, providing clear information about Beach Breeze Rehab and Care’s services, policies, and procedures.
• Liaise with physicians, hospitals, and other healthcare facilities to ensure a smooth and timely admission process.
• Educate residents and families about facility programs, services, and available resources.
• Provide emotional support and address concerns to foster a positive experience during admission.
• Maintain accurate and up-to-date documentation of admission assessments, care plans, and relevant information in resident medical records.
• Ensure compliance with regulatory requirements, facility policies, and documentation standards.
• Collaborate with the admissions team, nursing staff, and other healthcare professionals to streamline and enhance the admissions process.
• Participate in interdisciplinary team meetings to discuss resident care and admissions-related matters.

• Current and valid Registered Nurse (RN) or Licensed Practical Nurse (LPN) license in the state of Florida.
• Minimum of 2 years of nursing experience, preferably in admissions or long-term care settings.
• Strong assessment, critical thinking, and documentation skills.
• Excellent interpersonal and communication abilities.
• Knowledge of regulatory requirements related to nursing home admissions.
• Compassionate, patient-centered, and team-oriented approach to care.
