Palliative Care Coordinator

Overview

Trustbridge Hospice, a part of Empath Health, is seeking an organized and empathetic Care Coordinator to join our team in West Palm Beach
. This position is onsite

This full-time position works Monday through Friday, 8:30 a.m – 5 p.m., supporting our palliative care team and ensuring a seamless, empathetic experience for patients and families during one of life’s most meaningful transitions.

Since 1978,
Trustbridge Hospice has cared for more than 200,000 South Florida families. As a community-based nonprofit, we provide 24/7 hospice and palliative care, along with caregiver support and bereavement services for families facing serious illness.

• Coordinates and monitors services provided to patients and their families.
• Assists the Palliative care team in managing the patient’s care needs, communicating with patient care providers, and assisting the interdisciplinary team in contributing to patient’s plan of care.
• Demonstrates effective administrative activities to meet the requirements of our Palliative programs.
• Assists with communication and coordination of care with outside providers.
• Maintains patient confidentiality and discretion.
• Other duties as assigned based on organizational needs.

• An A.A. degree in Office Management, Medical Records, Business or a related field is preferred.
• Two years previous experience in medical/clinical setting required.
• Previous case management or care coordination experience preferred.
• Planning and research skills
• Must have a working knowledge of medical terminology
• Must be able to follow a defined list of tasks and follow the established procedures completing them in a timely fashion.
• Must have excellent computer skills (Excel, Windows, MS Office) with the ability to enter at least 40 CWPM and be willing and able to learn new computer software package.
