* Position Overview:

** The Hospital Liaison is responsible for conducting screens and assessments for newly delivered mothers and newborns to determine eligibility status for Health Beginnings Entry Agency referral services.  The Hospital Liaison effectively promotes program goals and communicates accurate information about the Healthy Beginnings program. This position will provide services in assigned hospital location(s) and may require scheduling flexibility in order to accommodate hospital schedules.

** This position works the schedule of Tuesday-Saturday 7:30am/8am-4pm/5pm.
**** Here’s what the job involves**:
* Conducts screening assessments with newly delivered mothers and infants in assigned hospital locations to determine eligibility for Healthy Beginnings services
* Collaborates closely with hospital staff, including Birth Registrars, to ensure timely and accurate completion of screenings
* Reviews and validates client information entered by Birth Registrars to ensure program eligibility and data accuracy
* Identifies risk factors, protective factors, and family needs using approved screening and assessment tools
* Serves as a liaison between hospitals, program participants, supervisors, and community partners
* Assists eligible families with access to program services, referrals, and community resources
* Completes required documentation and enters program data accurately and on time to support compliance and outcomes
* Participates in trainings, meetings, supervision, and program activities while remaining accessible via cell phone## Qualifications

* Qualifications:

*** Bachelor’s Degree in Human Services (social work, developmental psychology, and/or early childhood education). Bachelor’s degree in a different field with a minimum of 2 years’ experience with children and families.
* A minimum of 2 years in child welfare, primary prevention services, child development or related Human Services field (and Bachelor’s degree) preferred.
* Must be able to work Tuesday-Saturday
* Schedule includes occasional evening and weekend hours as needed
* Bilingual English/Creole required.
* Valid Florida’s driver’s license is required.
* Successful completion of a background check through Clearinghouse ()
