Trustbridge Hospice, a part of Empath Health, is seeking a compassionate and customer-focused Admissions Coordinator
to join our team in West Palm Beach
. This position is onsite in a call center environment.
This full-time position works Monday through Friday, 12:00 p.m. – 8:30 p.m., supporting the admissions process and ensuring a seamless, empathetic experience for patients and families during one of life’s most meaningful transitions.
Since 1978, Trustbridge Hospice has cared for more than 200,000 South Florida families. As a community-based nonprofit, we provide 24/7 hospice and palliative care, along with caregiver support and bereavement services for families facing serious illness.
What You’ll Do- Facilitate the conversion of referrals to admissions through completion of the intake process.
- Provide exceptional customer service to patients, families, and referral sources with empathy and professionalism.
- Communicate effectively with interdisciplinary team members to ensure timely and accurate admissions.
- Maintain accurate records and documentation in accordance with organizational policies.

- Three (3) years of experience in healthcare administration or customer service.
- Knowledge of hospice or home health programs preferred, with willingness to develop clinical understanding.
- Ability to handle a high call volume.
- Strong communication, organization, and computer skills.
