We welcome physicians with Value-Based Care (VBC) experience as well as those eager to learn.
Compensation & Incentives
- Base Salary: $,000 (based experience & location)
- Competitive sign on bonus
- Annual Performance Bonus: 20% of base salary

- 1215 patients per day
- Integrated Care Team:
- NP/PA
- Medical Assistants
- Social Workers
- Lab & Pharmacy support
- Referral coordination
Requirements
- Board Certified / Board Eligible in Internal Medicine or Family Medicine
- Geriatric experience preferred
- HEDIS experience a plus
- Value-Based Care experience preferred but not required; training provided
