The Community Health Worker will maintain a presence in the community with patients to promote, motivate and improve healthy living. The CHW will assist patients in utilizing resources and accessing services to overcome barriers in order to meet their health goals.

The above reflects the anticipated hourly wage range for this position if hired to work in New Jersey. The compensation offered to the candidate selected for the position will depend on several factors, including the candidate's educational background, skills and professional experience.

• Meet patients in a clinic, hospital, ER, and where patients are located; conduct needs assessment using the Social Determinants of Health Tool.
• Schedule weekly follow-up calls and home visits to patients.
• Help patients with social issues such as homelessness, substance abuse, hunger, transportation, and other social problems impacting members’ lives.
• Assist patients with organizing records and making follow-up appointments with Primary Care Physicians or Specialists.
• Help patients with tasks related to obtaining medical care and working toward health goals, including planning healthy meals and finding time to exercise.
• Help patients access resources, including quality of care, childcare services, food, housing, and transportation.
• Maintain relationships with Community Based Organizations.
• Able to travel and meet members where they are, which may include local churches, homeless shelters, or members’ residences.
