Under the direction of the department’s Director of Mental Health, the Mental Health Facilitator organizes, performs and coordinates the delivery of mental health services to designated patients. Provide high quality, compassionate and ethical care to patients in outpatient setting. Provide resources, referral to patients, parents and family members involved in care. Communicate/follow-up with collaborating agencies (DCPP, schools, etc) Respond to crisis situations when necessary
BS/BA in Psychology required. MSW, MA, Social Service or Social Work preferred. Valid NJ License Social Worker. 1-2 year of experience in an Outpatients Mental Health setting. B-lingual (English/Spanish)

Pay: $52,000.00 – $62,000.00 per year
Must be fully vaccinated against COVID-19
