About Us:
Southwestern Nursing and Rehabilitation Center is dedicated to providing exceptional, compassionate care to our residents. Our in-house therapy team works collaboratively to help residents regain function, improve quality of life, and achieve their highest level of independence.
We are seeking a PRN Speech-Language Pathologist (SLP) to join our dynamic therapy team. The SLP will assess, diagnose, and treat residents with speech, language, cognitive-communication, and swallowing disorders. This position plays a key role in supporting residents recovering from illness, injury, or managing chronic conditions.
Key Responsibilities- Conduct comprehensive evaluations of speech, language, voice, cognition, and swallowing functions.
- Develop and implement individualized treatment plans tailored to each resident’s needs and goals.
- Provide direct therapy services using evidence-based practices.
- Document evaluations, progress notes, and discharge summaries in accordance with facility and regulatory standards.
- Collaborate with interdisciplinary team members (OT, PT, nursing, social services, etc.) to ensure coordinated, holistic care.
- Educate residents, families, and staff on communication strategies and safe swallowing techniques.
- Participate in care plan meetings and provide professional recommendations as needed.
- Adhere to all state, federal, and facility policies and guidelines.
- Master’s degree in Speech-Language Pathology from an accredited program.
- Current SLP license in the state of practice.
- Certificate of Clinical Competence (CCC) from ASHA preferred.
- Prior experience in a skilled nursing or rehabilitation setting is highly desirable.
- Strong clinical assessment, communication, and documentation skills.
- Ability to work independently and effectively manage a flexible PRN schedule.
