We are seeking an experienced Case Manager RN to join our team for a travel assignment in California. This role requires strong inpatient case management expertise, knowledge of HMO and Medicare guidelines, and proficiency in Epic charting. The Case Manager RN will play a critical role in coordinating patient care across inpatient, home health, and skilled nursing facility (SNF) settings.
Responsibilities- Provide comprehensive inpatient case management services.
- Coordinate patient care transitions across hospital, home health, and SNF settings.
- Ensure compliance with HMO and Medicare guidelines.
- Collaborate with physicians, nurses, and interdisciplinary teams to optimize patient outcomes.
- Document patient care and case management activities using Epic EMR.
- Float within scope to meet facility needs as required.
- Experience:
Minimum of 3 years inpatient case management experience. - Licensure:
Active California RN license (pending licenses not accepted). - Knowledge: HMO and Medicare for inpatient, home health, and SNF settings.
- Charting:
Epic experience required. - Certifications:
BLS required. - Travelers:
First-time travelers accepted. - Locals:
Accepted. - RTO Restrictions:
No RTO permitted during the first 2 weeks of assignment.
