Job SummaryThe Supervisor of Care Coordination provides leadership and supervision to Care Coordination staff related to utilization management and discharge planning, ensuring the efficient delivery of high quality, cost-effective patient care. This role oversees daily operations, including denial management, performance improvement initiatives, and appeals processes, while maintaining compliance with organizational and regulatory standards. The supervisor promotes interdisciplinary collaboration among internal teams and external partners to optimize care transitions, resource utilization, and patient outcomes.
Additionally, the position actively participates in quality improvement programs, monitors departmental performance metrics, and identifies opportunities for enhanced clinical and operational efficiency.
- Minimum Education Requirement: BS/BSN preferred.
- Minimum Experience Requirement: Previous RN supervisory experience preferred. Two (2) years of experience is required in specialty area. Excellent customer service skills.
- Minimum License Requirement: California RN license. BLS required. Annual 1 hour of stroke education required.

Pay Range: $69.72 - $108.06
