Overview
SOCIAL WORKER (LBSW OR LMSW)
POSITION:
Social Worker. The qualified applicant should hold a Bachelor's or Master's degree in social work from an accredited program and have knowledge of the case management process and needs of geriatric patients, including funding resources. - Complete a social history and psychosocial assessment for each resident that identifies social, emotional, and psychological needs.
- Work with individual patients and families toward adjustment to center life and crises of illness, disability, and death.
- Advocate for patients and their families in expressing, defining, and resolving grievances.
- Report to the Director of Social Work Services the current status of social services and present needs.
- Participate in developing a written, interdisciplinary plan of care for each resident that identifies psychosocial needs/issues, goals, and appropriate social worker interventions.
- Provide therapeutic interventions to assist residents in coping with transitions and adjustments to a long-term care facility.
- Provide support and education to residents/family members to assist understanding of placement and facility issues, and refer to appropriate social service agencies when needed.
- Provide clinical interventions to address catastrophic events during the resident’s stay.
- Coordinate the resident discharge planning process and make referrals for home care services before return to the community.
- Maintain confidentiality of professional information.
- Develop and maintain a listing of current community resources helpful to residents and families.
- Participate in quality assurance interdisciplinary team meetings.
- Understand and meet government requirements for social service documentation.
- Document progress in meeting the psychosocial needs of residents.
- Work with the interdisciplinary team to promote resident rights and psychological well-being; prevent and address resident abuse as mandated by law and licensure.
- Identify community changes and opportunities such as legislation, regulations, and programs affecting nursing home residents.
- Collaborate with residents, families, staff to support self-advocacy and improve quality of life and care within the facility and community.
- Licensure: LSW, LBSW, or LMSW.
- Experience:
Prior work experience in a community or long-term care setting is preferred. - Skills:
Strong communication, assessment, and social work methods; ability to work with diverse disciplines; understanding of social systems; ability to work in a fast-paced environment. - Knowledge:
Understanding of medical case management, geriatric patient needs, healthcare settings, and funding resources (Medicare Part A/B/C/D, Medicaid, Managed Care, and other private insurance). - Other:
Insurance authorization, medical terminology, and marketing experience are preferred; ability to interact with potential admissions in a hospital setting; multitasking.
