Job Summary: The Housing Case Manager
- Special Project provides trauma-informed, survivor-centered case management, advocacy, and housing support to individuals and families impacted by domestic violence, dating violence, sexual assault, and stalking. This position supports survivors residing in DVCCC's emergency shelter, bridge housing, and transitional housing programs, ensuring coordinated access to housing, safety planning, legal advocacy, and wraparound services that promote stability, self-sufficiency, and long-term independence.

This position is supported by multiple funding sources. Responsibilities are outlined below by grant-funded program area to ensure clarity and compliance with grant requirements.

Grant-Funded Responsibilities

Emergency Shelter & Transitional Housing Case Management Services (50% FTE). William Penn Foundation/OIC

The employee will dedicate 50% of total work time to the following responsibilities:

  • Provide intensive, trauma-informed case management to shelter and housing residents, focusing on safety, housing stability, advocacy, and self-sufficiency.
  • Assist survivors in locating, securing, and maintaining transitional and permanent housing.
  • Conduct intakes, needs assessments, eligibility screenings, and housing evaluations for shelter admission.
  • Develop individualized case management and safety plans, identifying barriers and goals.
  • Conduct regularly scheduled client meetings to assess progress and adjust service plans.
  • Prepare shelter spaces for incoming residents and provide orientation, including policies, procedures, and tours.
  • Coordinate emergency financial assistance (e.g., food, transportation, clothing, housing-related costs, safety measures).
  • Liaison with Chester County OIC (Opportunities for Industrialization Centers) services to assist survivors achieve sustainable employment and financial independence. Monitor and coordinate education, training and supportive services to assist survivors build skills leading to self-sufficiency.
  • Educate clients about Victims Compensation Funds, Relocation Assistance, Moving Ahead, and other financial resources.
  • Prepare discharge summaries and transition paperwork upon program exit.
  • Maintain accurate client records in ETO, HMIS, Shelter/Bridge intake and exit systems.
  • Regularly assess client safety and well-being.
  • Facilitate or co-facilitate support groups, psycho educational groups, and housing-focused groups.
  • Assist with daily operations of the housing program.
  • Participate in hotline backup and on-call rotation as needed.
  • Attend case conferences with Chester County Open Hearth, Housing Authority of Chester County, and other partners to support housing outcomes.
Transitional Housing Project Coordination (OVW-Funded) (50% FTE)

The employee will dedicate 50% of total work time to the following responsibilities:
  • Serve as a consistent point of contact for transitional housing participants.
  • Conduct eligibility screenings and assist with program applications and enrollment.
  • Provide case management, supportive services, and crisis counseling to program participants.
  • Deliver program orientations to ensure participants understand available services and expectations.
  • Coordinate supportive services including transportation, counseling, childcare, healthcare, and other identified needs.
  • Assist participants with securing employment and workforce support.
  • Collaborate with clients to develop individualized support and housing stability plans.
  • Conduct ongoing needs assessments and regular check-ins to support successful housing outcomes.
  • Provide financial literacy education through the Moving Ahead program or community-based resources.
  • Deliver direct services including safety planning, emotional support, and crisis intervention.
  • Monitor participant progress and adjust strategies to promote sustainable independence.
  • Refer participants to DVCCC legal counsel and legal support staff for protection from abuse orders and other legal remedies.
  • Build and maintain partnerships with community agencies, shelters, housing providers, and service organizations.
  • Coordinate timely referrals to internal and external resources.
  • Track service delivery and client outcomes to support evaluation, reporting, and continuous improvement.
  • Complete required documentation, statistical reports, and quarterly progress and challenge reports in compliance with OVW and other funding requirements.
  • Ensure emergency shelter and bridge/transitional housing units meet cleanliness, organization, and compliance standards.
