Your Role:
Conduct timely assessments of patients in hospital to plan safe and effective discharges.
Work closely with multi-disciplinary teams to coordinate care and support for patients transitioning from hospital to home or other care settings.
Provide immediate guidance, support, and interventions to ensure smooth discharges while maintaining patient safety and well-being.
Collaborate with external agencies, care providers, and community services to meet patient needs.
Use your strong interpersonal skills, empathy, and commitment to patient-centered care to make a real impact.
Help reduce hospital stays and support individuals to regain independence in their own homes whenever possible.

What We Need:
You must hold a Social Work Qualification and be registered with SWE.
Previous experience in hospital social work or discharge planning is highly desirable.
