Overview
Sandwell Metropolitan Borough Council is looking to recruit Social Workers for a Borough wide Hospital & Intermediate Care Team. We operate a 7‑day service and work Bank Holidays. This is an exciting opportunity within the Hospital 2 Home team for individuals who are flexible, innovative, and have a proven track record of partnership working.
Responsibilities- Support the timely discharge of people from hospital into intermediate care settings.
- Work in partnership with NHS, ICB, and other agencies to promote independence, choice and control.
- Use strengths‑based approach and deliver care tailored to individual assessed needs.
- Undertake safeguarding investigations, mental capacity assessments, best‑interest decisions, outcome‑focused assessments, and support planning for vulnerable adults and their carers.
- Provide asset‑based and strength‑based support to achieve outcomes within the wider community.
- Qualified Social Worker (Social Work degree, Dip SW, CQSW or equivalent).
- At least six months of experience in a Social Care environment with adults.
- Experience working with older adults, people with physical disabilities, sensory impairments and mental health issues.
- Knowledge of current legislation including the Care Act 2014, Safeguarding, Continu‑Health Care, Mental Health Act, Mental Capacity Act 2005, and Community DOLS/LPS.
- Motivation, flexibility, and sound professional standards.
