Title:

Dialysis Chronic Care Registered Nurse (RN). This position requires 12 months or more of recent dialysis experience.

Purpose and scope

This position is accountable and responsible for the provision and coordination of clinically competent care including assessment, planning, intervention and evaluation for an assigned group of patients. This may include delegation of appropriate tasks to direct patient care staff including but not limited to RNs, LVN/LPNs and Patient Care Technicians. As a member of the End Stage Kidney Disease (ESKD) health care team, this position participates in decision-making, teaching, leadership functions, and quality improvement activities that enhance patient care outcomes and facility operations.

Principal

duties and responsibilities
  • All duties and responsibilities are performed in accordance with Client policy, procedures, standards of nursing practice, and state and federal regulations.
  • Perform essential functions under the direction of the Supervisor and with guidance from the Clinical Educator, Preceptor or in collaboration with other Registered Nurses.
  • Collect and analyze patient data pre-, during, and post-hemodialysis treatment; document in the patient medical record; adjust treatment plan as indicated; notify Team Leader, Charge Nurse, Supervisor or Physician as needed.
  • Assess, collaborate, and document patient/family learning needs to provide initial and ongoing education to patients and family.
  • Direct and provide, in collaboration with direct and ancillary staff, all aspects of safe and effective delivery of hemodialysis therapy to assigned patients.
  • Administer medications as prescribed or per approved algorithms; document justification and effectiveness.
  • Initiate or assist with emergency response measures.
  • Serve as a resource for the health care team and participate in staff training and orientation of new staff as assigned.
  • Ensure correct laboratory collection, processing and shipping procedures; reschedule missed or insufficient laboratory collections.
  • Identify expected outcomes, document and update nursing assessment and care plan in collaboration with the Interdisciplinary Team.
  • Educate patient on transplant and treatment modality options.
  • Perform other duties as assigned by Supervisor.
Physical demands and working conditions

Direct patient care with heavy lifting and moving of patients; frequent standing and bending. May move equipment up to 200 lbs with assistance; may lift chemical and water solutions up to 30 lbs to heights up to 5 feet. Work environment typical of a health care facility with controlled temperature and moderate noise. Potential exposure to infectious materials. Travel to training sites or other facilities may be required.

Ishihara color blindness testing is required; reasonable accommodations considered.

Supervision

Oversee Patient Care Technicians/LPNs/LVNs/RNs as a Team Leader or Nurse in Charge after meeting training requirements and having: a minimum of 9 months RN experience and a minimum of 3 months chronic/acute hemodialysis RN experience.

Education and licensure
  • Graduate of an accredited School of Nursing.
  • Current appropriate state licensure.
  • Current or successful completion of CPR/BLS certification.
  • Must meet practice requirements in the state of employment.
Experience and required skills

1 year of Nephrology nursing experience in the last two years as an RN; chronic hemodialysis experience required.

Floating and shift details

Floating requirement: Candidates may float to area sites within 50 miles of the main site.

Shift details: Shifts discussed during interview; role requires 40 hours plus overtime and floating as needed. There is a minimum guarantee of 36 hours per week. Not eligible for guaranteed hours if schedule requests/restrictions, refusal to float, lateness, leaving early, calling out, or planned time off.
