Overview
Professional Services Group is hiring for the Care Coordinator with our CCS/CLTS Dual Care Coordination program in Milwaukee County. This role supports children and families facing severe mental health and/or substance use disorders with co-occurring developmental and/or physical disability needs. The program follows the wraparound model of service delivery to identify needed services and develop multi-disciplinary treatment plans that address both strengths and needs of the family.
The Care Coordinator will manage a caseload of youth dually enrolled in Comprehensive Community Services (CCS) and Children's Long-Term Support (CLTS) programs and coordinate in-home and community-based services to support individualized family needs.
- Provide case management services to a caseload of 12-13 clients.
- Interview and evaluate clients and families during intake and assessment to analyze needs and strengths.
- Create and implement goal-oriented, strengths-based treatment plans.
- Facilitate team meetings with the client, family, and direct service providers to monitor progress.
- Develop a crisis/safety plan for the client and family.
- Develop a family support team comprised of formal and informal supports.
- Collaborate with other providers, school personnel, mental health professionals, Child and Family Services workers, and court officials.
- Intervene as an advocate for clients in emergency and crisis situations as needed.
- Safely transport clients as needed.
West Allis, WI. Local travel is required to meet with clients within their home and other community spaces throughout Milwaukee County and surrounding areas, with mileage reimbursement.
ScheduleMonday–Friday 8:00am–5:00pm, with occasional evenings to accommodate families' schedules.
SalaryCandidates with a Bachelor's Degree: $ per hour
Candidates with a Master's Degree: $ per hour
