Medical Transition Specialist

Patient Engagement Advisors (PEA) is seeking a full‑time Transition Specialist to work onsite at Advent Health Wesley Chapel. The role also serves as an alternate Logistics Transition Specialist
, performing Logistics Courier duties within the local area.

Schedule
:
Monday – Friday 8:00 AM – 4:30 PM, onsite at Advent Health Wesley Chapel and local area for courier duties.

As a Transition Specialist, you will set patients up for success to transition out of the hospital into their next stage of care, ensuring they receive services, are educated on discharge requirements, and have the resources needed for the next stage. The Logistics Transition Specialist delivers medications, nutrition, product kits, and other patient‑care items to homes or other hospital locations.

Responsibilities
  • Schedule patients’ follow‑up appointments
  • Schedule patients’ transportation as needed
  • Deliver discharge prescription medications to patients’ bedside
  • Assist patients with medication affordability
  • Coordinate home meal delivery service for patients
  • Connect patients to community resources as needed
  • Deliver medication, nutrition product kits, and other patient‑care items to patients’ homes or other hospital locations
  • Collect and document social determinants of health information for every patient to better understand patient needs and provide optimal services
  • Provide pick‑up and delivery of prescriptions and medical supplies accurately and timely
  • Ensure proper handling and storage of all medications and supplies
  • Obtain signature of person receiving the delivery
  • Collect payments; comply with payment collection policies and procedures
  • Utilize PEA software and assistance programs (coupons, vouchers, discount cards) to offer optimal savings/value for patients
  • Utilize Countdown to Discharge (C2D) and other applications appropriately to document completed activities and fulfill patient needs
  • Meet KPI “Scheduled” goals and ensure follow‑up appointments are documented in C2D
  • Actively participate in hospital‑specific rounds/huddles to obtain critical information and contribute as a collaborative member of the clinical care team
  • Submit one “Wow Story” based on PEA Leadership’s requested intervals to Supervisor describing exceptional patient care experience provided

Requirements
  • High school education or equivalent required
  • Must be 18 years old or older
  • 6 months pharmacy technician experience (retail preferred) OR 1‑year medical assistant / patient care technician / patient care coordinator / clinic front desk associate / nursing assistant / home health aide / other healthcare or medical experience required
  • Retail pharmacy experience strongly preferred
  • Other related healthcare, hospital, or medical experience will be considered
  • Associate’s or Bachelor’s degree will be considered in the absence of healthcare experience
  • Ability to report to work onsite as scheduled; ability to be on feet – standing, walking, and/or climbing stairs 8‑10 hours daily
  • Valid driver’s license in the state of employment with a clean driving record
  • Ability to receive required vaccinations (as required by PEA or hospital) unless approved for exemption
Knowledge, Skills & Abilities
  • Ability to work a flexible and/or rotating schedule, including nights, weekends, and holidays
  • Excellent interpersonal skills; ability to connect with patients/families and multidisciplinary providers
