Wellsential Health is seeking an experienced Registered Nurse (RN) to serve as Assistant Director of Nursing (ADON) at Weslaco Nursing and Rehabilitation Center

The Assistant Director of Nursing (ADON RN) plays a vital leadership role within the Skilled Nursing Facility (SNF), partnering with the Director of Nursing (DON) to ensure clinical excellence, regulatory compliance, staffing stability, and quality resident outcomes. This role is ideal for a strong Nurse Leader ready to advance toward a Director of Nursing (DON) position within a mission-driven, nonprofit organization.
Key Responsibilities Include:- Assist the Director of Nursing (DON) in planning, organizing, and directing daily nursing operations in compliance with CMS regulations, Texas HHSC standards, and facility policies.
- Promote continuity of care by coordinating nursing services with interdisciplinary teams, physicians, residents, and families.
- Support survey readiness efforts and ensure regulatory compliance across all shifts.
- Participate in recruiting, hiring, onboarding, training, and retention of nursing staff.
- Develop staffing schedules and assignments to maintain appropriate coverage and safe resident-to-staff ratios.
- Monitor clinical performance, documentation accuracy, and care planning processes.
- Support implementation and monitoring of the Quality Assurance and Performance Improvement (QAPI) program.
- Assist with MDS coordination, reimbursement optimization, and clinical documentation review.
- Ensure compliance with infection control standards, medication management protocols, and best practices in long-term care.
- Review audits, incident reports, and consultant findings; develop and implement corrective action plans as needed.
- Participate in the facility’s on‑call rotation and perform additional duties as assigned by the Administrator or Director of Nursing.
- Active and unencumbered Registered Nurse (RN) license in the State of Texas.
- Minimum of three (3) years of clinical experience in a Skilled Nursing Facility (SNF) or Long‑Term Care (LTC) setting.
- Prior nursing leadership experience (Charge Nurse, Unit Manager, ADON, Nurse Supervisor, or similar role) preferred.
- Strong knowledge of CMS regulations, Texas HHSC survey process, QAPI, MDS, care planning, and regulatory compliance.
- Current CPR certification.
- Must be eligible to work in the United States.
