SummaryThe Nurse Navigator is accountable for navigation and coordination of care utilizing evidenced-based clinical pathways for cardiology patients along the continuum of care for their cardiac condition from diagnosis through treatment. They will provide longitudinal comprehensive nursing care to a designated case of cardiac patients. They will have primary nursing responsibilities for the coordination of care, symptom management and education of patients and families.
This position will work in collaboration with all members of the healthcare team to achieve the best possible outcomes for the patient.

QualificationsRequired:
- Current licensure in the state of Washington (RCW 18.88) or licensure through Multistate Nurse Licensure Compact (SSB 5499).
- Current BLS Health Provider and ACLS card.
- Competent in basic computer skills.
- 3 years of clinical experience.
- Ability to work independently and perform role with minimal guidance
Desired:
- Previous department experience.
O = Occasional, represents 1 to 25% or up to 30 minutes in a 2 hour workday.
F = Frequent, represents 26 to 50% or up to 1 hour of a 2 hour workday.
C = Continuous, represents 51% to 100% or up to 2 hours of a 2 hour workday.
