Summary

This is a pool, as needed position with no guarantee of hours. A clinically competent, licensed practical nurse who delivers care as a team member to the acutely ill patient under the direction of an RN. The LPN is knowledgeable in the nursing process of assessment, nursing diagnosis, outcome identification, planning, implementation and evaluation, and effectively interacts with patients, significant others, and other health team members while maintaining standards of professional nursing.

Functions
  • Provides care to acutely ill patients, using a team approach.
    • Follows the plans of care as developed by the RN and contributes data to the RN in the development of the plan of care.
    • Develops and maintains effective communication with the primary nurse, physician, patient, family, and other members of the health‑care team.
    • Seeks direction in the delivery of patient care and informs the designated RN of current developments and problems, utilizing the proper communication system.
  • Assists in assessment of patient’s condition, reactions, and in planning individual care.
  • Documents and reports pertinent information, including medications and nursing care plans.
  • Measures and records vital signs, I & O, weights, and administers treatments, including sterile procedures.
  • Assists other disciplines in evaluation and implementation of specific goals for the patient, including physical, occupational, speech, recreational therapy, and dietary.
  • Participates in patient education, including ADLs, skin care, medication, and disease process (CVA, diabetes, UTI, etc.).
  • Reviews and implements physician’s orders under direction of the primary RN and administers medication, IVs if certified in‑house.
  • Renders basic nursing care, accurately charting same.
  • Provides care for patients, demonstrating necessary skills and knowledge according to Nursing policy, unit specific competencies, and Standards of Care.
  • Exhibits interpersonal skills that promote a positive functioning department, including effective communication with peers, health team members, management, and physicians.
  • Accurately and concisely processes information and communicates appropriate information to the Primary Nurse, physicians, and other health team members in a timely manner.
  • Maintains a safe and sanitary work environment, complying with specific hospital and departmental policies regarding safety and disaster plans.
  • Collaborates with hospital personnel and physicians, communicating patient needs, nursing assessments, and recommendations daily.
  • Adheres to Hospital/Nursing Policy, observing guidelines concerning unscheduled absences, overtime, tardiness, dress code, personal hygiene, and infection control.
  • Assumes responsibility for self‑evaluation and improvement, attending mandatory in‑services, CPR certification, and staying current with the Washington Nurse Practice Act.
Qualifications

Required:

  • Graduate of an approved LPN program or enrolled in an RN program.
  • Current nursing license in the state of Washington.
  • Completion of hospital orientation program.
  • Must be currently certified in CPR and recertify annually.

Working Conditions

Applies principles of safety in daily work. Service requires patient care 24 hours a day, seven days a week, thus scheduling involves rotation of days with possible rotation of shifts for vacation, illness, etc., and occasional overtime as needed. The LPN may be asked to float to other units to provide balanced staffing or assist as needed.
