Responsibilities- Assures that the clinic maintains standards of pharmacy practice equal to or above those accepted by the profession and the law.
- Performs the duties of a pharmacist, including dispensing of prescriptions, counseling of patients and medication therapy management.
- Utilizes pharmacy software and technology systems effectively, including competency surrounding patient safety measures built into such systems.
- Participates in the process/procedures of clinical pharmacist programs to enhance patient outcomes and optimize pharmacist knowledge and impact, including the use of prescriptive protocols and therapeutic substitution policies.
- Collaborates with pharmacists and other healthcare providers to ensure safe and cost‑effective medication therapy for clinic patients.
- Contributes to the achievement of the clinical and sustainability goals of the department.
- Provides in‑service training for pharmacy personnel or other clinic personnel as related to pharmaceutical issues.
- Provides pharmaceutical information, as needed and requested, to other healthcare providers.
- Participates in quality assurance programs and projects, clinic provider team discussions, and clinic‑wide committees; assists with supervision of pharmacy technicians, assistants, and students; ensures compliance with safety and confidentiality requirements.
- Assists with training and orientation of new pharmacy employees or students.
- Manages outpatient anti‑coagulation therapy via prescriptive protocol, including patient education, dose adjustment, monitoring, and triage of side effects.
Performs other duties as assigned by supervisor; maintains attendance standards; follows safety policies; demonstrates sound work ethic and commitment to core values; ensures compliance with CVCH policies, Joint Commission, and HIPAA regulations.
Job Specifications- Education: Bachelor of Science in Pharmacy or Pharm
D; PGY1 Community Pharmacy or PGY2 Ambulatory Care residency preferred. - Certification / Licensure: Washington State licensed pharmacist;
Board of Pharmaceutical Specialties certification preferred;
Basic Life Support required. - Experience: Experience in clinic pharmacy setting; knowledge of ordering, inventory, cost management; personnel management experience preferred; at least two years community pharmacy experience preferred.
