Salary Range: $24.00 To $26.00 Hourly
Primary Responsibilities
- Work with Veterans on a variety of issues to assist in identifying and overcoming challenges accessing the healthcare system or adhering to recommended health care plans
- Assist Veterans in accessing healthcare systems by gaining entry to VA health care (including mental health care) or community care when Veterans are not eligible for VHA
- Connecting Veterans to VA health care by working with the VAMC to facilitate enrollment, helping to gather documentation and complete required paperwork and following up on enrollment progress to ensure that the Veteran is enrolled in VA or community health care services
- Help Veterans get access to appointments when needed by supporting Veterans in identifying health care needs and working collaboratively with health care teams to facilitate access to care
- Assist Veterans in utilizing available services including preventative health care by communicating with Veterans and health care teams about appointments
- Identify barriers to adhering to recommended health care plans
- Assist Veterans in understanding and communicating with providers to make informed decisions about health care by supporting and encouraging Veterans to discuss questions about medication or treatment goals with providers
- Problem-solve barriers to care (i.e. transportation, childcare)
- Provide education for Veterans to learn about wellness related topics through printed materials, inviting guest speakers to education groups on health-related issues for Veterans, and linking Veterans to support groups or other programs at the VA or in the community to support their health goals
- Responsible to understand coding for PO's and to properly code and allocate activity reporting

Secondary Responsibilities
- Assist participants in building life and fiduciary skills to create housing sustainability
- Conduct outreach in the community and with network providers
- Perform initial client intakes and program screening, utilizing a Housing First model
- Provides services to assist participants in developing needed skills of daily living and housing stabilization
- Provides and documents referrals to any applicable resources
- Works with participants to obtain and become stable in permanent housing
- Provides liaison role between property owners/managers and program participants
- Ensures that participants have access to available housing resources
- Ensures that participants can pay rent, utilities and manage affairs necessary to remain stable in their housing
- Helps participants build the skills to keep their living quarters clean and safe.
- Works with participants to obtain relevant vocational training
- Responsible for coordinating with service agency's monthly developing working relationships
- Certified Family Development Specialist or acquired within one year of employment
- Effective communicator with the ability to build and maintain relationships within and beyond the organization
- Ability to meet deadlines and follow through on assigned tasks
- Ability to work with individuals of diverse backgrounds and disabilities, including handling difficult situations and diverse cultural backgrounds, with professionalism and compassion
- Proficiency in Microsoft Suite and various other computer software programs
- Prefer undergraduate degree in related field or equivalent experience with minimum of one-year successful experience working with individuals or families requiring assistance
- Ability to respond appropriately in crisis situations
- Ability to perform daily duties to achieve expected outcomes and performance measures with minimal supervision
- Ability to travel to various sites daily if needed, with vehicle or mileage provided
