Provides quality, cost-effective healthcare under the supervision of a registered nurse or licensed practical nurse. Emphasis shall be placed on maximizing patient’s functional abilities in the activities of daily living; promoting, maintaining and restoring health, preventing and minimizing the disabilities of acute and chronic illness. Discharges duties in a responsible professional manner while demonstrating values of respect, dignity, integrity, and compassion.
Actions are consistent with hospital policies and procedures and current standards/regulation governing nursing practice and long term care.
Minimum Qualifications
- High School diploma or equivalent.
- Current Certified Nursing Assistant Certification through the West Virginia DHHR.
- CPR certification.
Experience
- Three (3) months experience as a nurse assistant.
CORE DUTIES AND RESPONSIBILITIES
- Follows established plan of patient care.
- Assists with the admission, transfer, discharge and transporting of all patients.
- Implements patient safety measures, including but not limited to:
- Securing patients appropriately for transport.
- Assisting patient with positioning, transfers and ambulation.
- Ensuring call light is within easy reach.
- Collects and records vital signs as instructed by the registered nurse and licensed practical nurse.
- Provides measures to meet patient's comfort needs.
- Collects and prepares specimens according to policy and procedure including, but not limited to urine, stool, and/or sputum.
- Administers care and treatments for patients as instructed by the Registered Nurse, including but not limited to:
- Weighs patient
- Gives enema
- Provide activities of daily living/restorative care
- Performs perineal care
- Performs catheter care
- Performs oral hygiene
- Applies hot or cold compresses
- Strains urine
- Obtains T.P.R.,temperature, pulse and respirations
- Obtains intake and output
- Ambulates patients
- Turns and positions patients maintaining proper body alignment
- Provides sitz bath
- Provides pre‑operative care
- Provides post‑operative care
- Performs post‑mortum care
- Applies assistive devices, braces and equipment
- Restraint application and the identifications of restraint alternatives.
- Performs basic routine duties including, but not limited to:
- Distributes fresh water every shift
- Disposes of soiled linen every shift
- Maintains clean and orderly unit
- Maintains and stocks unit supplies
- Practices clean and sterile technique in the delivery of patient care while maintaining body substance isolation.
- Aware of the location of equipment and supplies on the nursing unit.
- Demonstrates knowledge and skill necessary to provide care and education appropriate to the age of the patients served. (Adult 18-79 years, Geriatric 80+ years)
Performs Food And Nutritional Duties Including
- Distribution of snacks and supplements
- Maintains fluid restrictions
- Assists patients with meals and supplements as needed.
- Documentation in the electronic medical record
- Provides care for the respite and hospice patient.
- Attends in‑services and other educational programs as required with emphasis on programs related to the geriatric patient.
- Attends and participates actively in a minimum of three staff meetings annually.
- Demonstrates proficiency, competence and current knowledge in providing patient care.
- Determines areas of self‑improvement
- Supports and adheres to hospital, nursing service and skilled care policies and procedures.
- Provides restorative patient care in a manner that promotes maintenance or enhancement of the patient’s quality of life.
- Participates in continuous improvement activities.
- Sets short term goals to organize work to meet patients needs.
- Institutes emergency measures calmly and effectively.
- Prepares and submits incident reports.
- Reports safety or risk management problems to Director and charge nurse or appropriate hospital personnel.
- Supports hospital and departmental cost containment efforts.
- Reviews and initials meeting minutes and postings in the department.
- Responds to patient’s call light immediately; notifies nurse of patient problems.
