• Assist with ADLs under RN supervision at the center and in participants' homes
• Contribute to care plan development and collaborate with the interdisciplinary team
• Perform non-skilled treatments like vital signs, transfers, toileting, bathing, dressing
• Help with recreational activities and assist therapists with treatments
• Maintain a clean and safe environment, assist with meals, and document care accurately
• Participate in infection control and team meetings

• Completion of an approved HHA or CNA training program preferred; work experience may suffice
• Certification as HHA or CNA desired
• Reliable transportation required
• At least one year of experience with frail or older adults preferred; less experience requires training
