Physical Therapist; PT​/PRN - Per Visit - Home Health

Frontpoint Health is a fast-growing home health agency committed to delivering personalized, high-quality care. We’re currently seeking experienced and compassionate Physical Therapists (PTs) to join our team and help patients recover safely and comfortably in their homes. Hiring in Weatherford, TX. Per visit rate. Sign-On bonus: $15,000 (Full-Time Only).

• Provide in-home physical therapy to patients recovering from injury, surgery, or illness
• Evaluate patients’ physical conditions and develop individualized care plans
• Implement therapeutic exercises and mobility techniques
• Educate patients and caregivers on rehabilitation best practices
• Collaborate with nurses, physicians, and care teams
• Complete timely and accurate documentation in our EMR (Kinnser/Well Sky)

• Active Texas Physical Therapist (PT) license (Required)
• 1+ year of home health experience (Required)
• Reliable transportation and valid driver's license (Required)
• Familiarity with Kinnser/Well Sky (Preferred)
• Excellent time management and communication skills
• CPR certification (Preferred)

• Sign-On Bonus: $15,000 for eligible full-time candidates
