Monday-Friday 8am-4:30pm.
Care Management Lead assists departmental leadership with daily operations and acts as a resource for Care Management staff. Performs work of care management role for the Care Management Department and is considered an expert in their field. Provides mentoring and education support to Care Management staff.
ExperienceA minimum of 3 years of applicable clinical practice experience as a Registered Nurse, and at least 2 years of experience in Care Management. Minimum of 3 years as RN. 5 years of clinical nursing and case management experience is preferred. Experience in case management field including transitional planning, outcomes management and assessment preferred. Experience in precepting or training in clinical setting, preferred.
3 years or more years of hospital based Care Management experience is preferred.
A Bachelor’s Degree in Nursing required, or a minimum of two years education at an accredited School of Nursing in order to be eligible for licensure as a Registered Nurse is required. General knowledge of payor industry (Medicare, Medicaid, & Commercial insurance regulations), resource management, and evidence based clinical practice is essential. Theoretical knowledge of nursing or social work, dependent on degree, and case management processes.
Analytical skills necessary to assess patient's discharge needs and develop solutions to problems concerning patients' mental status or physical well-being to ensure optimal patient outcomes. For RN applicants: bachelor's degree in nursing.
Accredited Case Manager Certification (ACM) or Certified Case Manager (CCM) certification. SPECIAL
SKILLS REQUIRED:
Highly organized with the ability to manage time and prioritize workflow. Ability to be self-directed and work autonomously. A high degree of positive energy and the ability to rapidly initiate positive relationships with others to develop trust. Demonstrates creative solution-based problem solving, critical thinking skills, effective conflict management and clinical judgment. Proficient in Microsoft applications and computer skills, required. Excellent communication, organizational, and leadership skills.
Positive and creative problem solving. Proficiency in EPIC. Skills otherwise required for department staff position.
Current state of Wisconsin Registered Nurse License or a Multi-state Nursing License from a participating state in the NLC (Nurse Licensure Compact). Wisconsin Registered Nurse License. Accredited case manager certification (ACM-RN) through the American Case Management Association (ACMA) is preferred.
