Position Overview:
The St. Camillus Home Health RN Case Manager position is responsible for managing and coordinating care for patients in their homes. This includes conducting assessments, developing individualized care plans, providing skilled nursing services, and ensuring that patient care is delivered in accordance with physician orders, agency policies, and regulatory guidelines. The RN Case Manager plays a central role in the interdisciplinary team, working to promote optimal health outcomes and patient satisfaction.
Key Responsibilities:
- Conduct initial and ongoing patient assessments to determine clinical needs and eligibility for home health services.
- Develop and revise individualized care plans based on patient condition and goals.
- Deliver direct nursing care including wound care, catheter care, and disease management education.
- Monitor and document changes in patient condition, communicate findings to the physician and team members, and adjust the plan of care accordingly.
- Coordinate and communicate effectively with physicians, therapists, home health aides, and family members to ensure holistic and continuous care.
- Educate patients and families on treatment plans, self-care techniques, medications, and disease processes.
- Ensure compliance with all local, state, and federal regulations, as well as agency policies and procedures.
- Maintain accurate and timely clinical documentation in the electronic medical record (EMR) system.
- Participate in case conferences, staff meetings, and quality assurance activities.
Qualifications:
- Current and active RN license in the state of Wisconsin.
- Minimum of 1–2 years of clinical nursing experience; home health experience strongly preferred.
- Knowledge of Medicare regulations and OASIS documentation standards.
- Current CPR certification.
- Valid driver's license, reliable transportation, and auto insurance.
