Overview
We are seeking an experienced Hospice Social Worker to join a compassionate hospice care team in Wauwatosa, WI. This role focuses on providing emotional, psychosocial, and resource-based support to terminally ill patients and their families during end-of-life care.
CompensationTravel Pay: $2,990/week
Local Pay: $2,740/week
Schedule- Monday – Friday
- 8:00 AM – 4:30 PM
- No on-call
- No holiday coverage required
- Patients with terminal diagnoses (prognosis of 6 months or less)
- Average caseload: 35–40 patients per social worker
- Conduct initial psychosocial assessments and routine hospice visits
- Support patients and families through the end-of-life process
- Assist with advance directives, Power of Attorney, and DNR discussions
- Connect families with appropriate community resources
- Provide emotional support, counseling, and guidance
- Collaborate with the interdisciplinary hospice care team
- Maintain accurate and timely documentation
- MSW or BSW required
- Hospice experience preferred
- First-time travelers not accepted
- Strong communication skills with empathy and end-of-life counseling experience
- Knowledge of community and hospice resources
