, as an Oncology Nurse Navigator at our Tinley Park clinic. This 100% onsite position requires you to be based in the greater Chicagoland area. Under the direction of clinical leadership and in collaboration with the program support team, you coordinate the care of patients through the disease process, acting as a clinical liaison across medical services and clinical departments, assessing and managing patients’ clinical, emotional, spiritual, psychosocial, financial, and other needs throughout treatment.
Job Functions
- Act as liaison for patients, families, and the multidisciplinary clinical team.
- Assess clinical, emotional, spiritual, psychosocial, financial, and other patient needs.
- Facilitate resolution of patient needs and monitor follow‑up.
- Educate and reinforce education with patients and families on disease, treatments, side effects, adverse reactions, and reportable signs/symptoms based on a multidisciplinary care plan.
- Coordinate discharge planning with the patient’s family, social workers, home nursing staff, oncologists, and personal physicians.
- Ensure discharge instructions are understood and followed.
- Develop or coordinate development of patient education materials, clinical pathways, and quality studies.
- Coordinate and participate in multidisciplinary consultations and/or tumor conference.
- Document recommendations made at multidisciplinary consultations and/or tumor conferences.
- Help develop a treatment plan that is understandable and feasible to the patient.
- Serve as clinical partner and escalation point for Intake Coordinators.
- Promote community education and increase prevention awareness and early‑detection screening programs.
- May be responsible for direct patient care.
- Perform other duties assigned.
- Registered Nurse with a current Illinois license.
- Graduate of a National League for Nursing accredited School of Nursing with a BSN.
- Minimum of two years of hematology/oncology clinical nursing experience with demonstrated competence in patient care, teaching, and management of patients with a cancer diagnosis; knowledge of oncology protocols and data management.
- General computer skills and adaptability to changing working situations and assignments.
- Excellent interpersonal skills.
- Oncology Certified Nurse.
- OCN Certification.
- ONS Chemotherapy Provider.
