Position

Lutheran Social Services of WI and Upper MI seeks to hire for the In-Home Recovery Services (IHRS) team as a Human Services Professional. This opportunity supports clients with significant mental health concerns enrolled in the Comprehensive Community Services (CCS) or Community Support Program (CSP) through Northcentral Healthcare in the Wausau community. This is a full-time, benefit-eligible position. Schedule is anticipated to be Tuesday - Friday, 2 PM – 10 PM and every other weekend.

Hours worked during evenings or on weekends include an additional $1.50/hour, bringing the hourly rate to $26/hour.

Responsibilities
  • Provide medication management support to clients in the home, school, office, or community under the direction of the Program Supervisor with assistance from Lead staff.
  • Demonstrate strong attention to detail and problem-solving skills.
  • Ensure all staff have a valid driver's license, automobile insurance, and reliable transportation to travel throughout the Wausau area.
  • Examples of services may include (these are examples, not all-inclusive):
  • Medication Monitoring - Non-Prescriber (e.g., deliver medications to improve compliance, review and teach clients about medications and potential side effects)
  • Participation on recovery teams to provide feedback on service delivery and progress
Essential Duties and Responsibilities

This list is not all-inclusive and may be expanded to include other duties as needed.

  • Provide services to assigned clients in alignment with the service plan.
  • Organize and plan work effectively.
  • Apply knowledge of community resources, counseling and casework principles, human behavior, group dynamics, interpersonal relations, and social problems and their effects on individuals.
  • Work collaboratively with clients, service teams, and clinical supervisor to determine interventions and coordinate services per the service plan.
  • Establish and maintain effective working relationships and complete required documentation within timelines per LSS, regulatory, funder, and practice standards.
  • Maintain client records and make recommendations within the client team and scope of practice.
  • Testify at legal proceedings as required by law and/or role; attend client/recovery team meetings; engage in clinical supervision when required; participate in staff development and training; provide peer consultation within scope of practice.
  • Maintain a flexible weekly schedule that includes some evening and/or weekend hours for client appointments and understand productivity expectations.
  • Perform other duties and special projects as required.
