Responsibilities- Clinical Documentation:
Maintaining accurate and thorough daily documentation of patient conditions, care provided, and responses to treatments in the resident record. - Medication & Order Management:
Transcribe physicians’ orders to MARs and promptly notify pharmacy of new orders. Safely prepare and administer medications as ordered by the physician, adhering to all state and federal regulations. - Patient Monitoring:
Conduct comprehensive clinical assessments, proactively monitoring patient status, identifying subtle changes, and initiating appropriate interventions promptly. - Team Supervision:
Provide clinical direction, supervision, and mentorship to LPNs and CNAs, ensuring the highest standard of patient care is delivered across the shift. - Interdisciplinary
Collaboration:
Participate in care planning, effectively communicating patient needs and goals to the entire interdisciplinary team (Therapy, Dietary, Social Services). - All other duties as assigned by the Director of Nursing, ensuring the smooth and compliant operation of the nursing department.
