Waukesha Memorial Hospital (Support at Outpatient Physician Center as needed)
- Schedule that supports work-life balance: 8:00 AM – 4:30 PM, no weekends/nights/holidays.
- Broad, multidisciplinary Neuroscience team caring for spine pain, concussion, neurological rehab, stroke follow-up, and complex neurosurgical and post-operative care.
- Impactful patient work:
Care coordination, triage, education, post-surgery follow-up, imaging coordination, rehab referrals, pain management, and neurological condition management.
- Spine care and minimally invasive spine procedures, spinal fusion, decompression.
- Pain management (non-surgical and post-surgical).
- Neurological conditions: concussion, brain injury, stroke follow-up, neuro-rehabilitation, nerve pain, chronic neuro conditions.
- Neurosurgical support for brain, spine and nerve disorders, plus post-operative management and coordination with neurosurgeons, rehabilitation, physical therapy, pain management, imaging, and patient education.
- Phone triage and clinic nursing support for neurosurgery patients (symptom assessment, medication refill coordination, prior authorizations, imaging/appointment scheduling, post-op follow-up and care coordination).
- Central resource educating patients and families on surgical preparation, recovery expectations, pain management, mobility precautions, imaging follow-up, and rehab referrals.
- Collaborate with multidisciplinary team (neurosurgeons, physiatrists, pain‑management specialists, therapists, APNs, navigators) to ensure seamless, patient‑centered care from diagnosis through post‑operative recovery and rehabilitation.
- Support both surgical and non‑surgical pathways: spine pain, neurological disorders, concussion/stroke follow‑up, rehab coordination, and long‑term neurological condition management.
- Day Shift: 8:00 AM – 4:30 PM
- No nights, weekends, or holidays
- Primary work site:
Waukesha Memorial Hospital Neuroscience Care clinic - Occasional support at Outpatient Physician Center (Oconomowoc) or PHC Medical Group clinic locations, as needed

What You Will Need- Associate's Degree in Nursing required. Bachelor’s Degree preferred.
- Wisconsin Registered Nurse License.
- Valid American Heart (AHA) Basic Life Support (BLS) certification.
