Erie Family Health Centers, a top workplace in Chicago and suburbs, is looking for a valuable addition to our Case Management team! The Case Manager will work with medical providers and nursing staff to identify potential clients, assess, and deliver supportive services to clients.

Duties & Responsibilities
- Conduct outreach, advocacy, assessments, care planning, home visitation, and EMR data entry.
- Collaborate with fellow Case Managers to provide constant floor coverage throughout the day.
- Display familiarity with each client’s care plan and facilitate referrals and follow-up processes.
- Follow the Department of Human Services Maternal and Child code requirements.
- Act as a mandated reporter to the Department of Children and Family Services.
- Education:
Bachelor’s Degree required in social work, child development, community health, or a related field. - Minimum of two years’ work experience in health, social or community services preferred.
- Bilingual (English/Spanish) required (may be required to assist or work with non-English/Spanish speaking patients and/or guests).
- Local travel required; valid Illinois Driver’s License and automobile insurance also required.
