The Community Health RN Team Lead provides leadership support and oversight to nursing staff and is responsible for implementing the day-to-day operations and patient flow for nursing care at the assigned clinical site.

• Triage assessment process of walk-in patients and telephone calls.
• Delivers health education related to medication, diet, treatment of diagnosis, health promotion, prevention and exit health teaching.
• Administers immunizations, applicable contraception refills, and medications as ordered by provider.
• Provides guidance and support to nursing staff at assigned sites.
• Provides for regular nursing team meetings and 1:1 supervisory meeting with direct reports.
• Other duties as required.

Education

• BSN required, master’s in public health or related field desirable

Skills And Knowledge

• Required: Current BLS for Healthcare Provider CPR certification

• Preferred: Minimum three years of experience in community health nursing or ambulatory care preferred
• Supervisory experience preferred
