Position: Nurse Case Manager (RN) - IBCCP Program

Overview

The Nurse Case Manager is responsible for working with Service Providers within and outside of the Health Department to facilitate patients access to care in accordance to Illinois Breast and Cervical Cancer Program (IBCCP) guidelines. Coordinates and assist Assistant Nurse Coordinator to provide education, information, and access to human services available in the community.

    Hours:

    37.5 hours per week
  • Monday to Friday (9:00AM - 5:00PM) – be a liaison between LCHD/CHC and outside providers of care.
  • Assist in coordination of Primary Care services with other Health Department programs and community agencies.
  • Participate in identifying client needs and make appropriate referrals.
  • Follow up on referrals of IBCCP clients, track and monitor test results by contacting the hospitals or the providers.
  • Provide clients with health education, laboratory and consultation results and recommendations explained in a language appropriate to the client’s level of understanding.
  • Maintain client records, documenting interactions, services provided, and phone calls.
  • Participate in CQI activities to assess and monitor client care processes and develop improvement plans when necessary.
  • Demonstrate critical thinking skills in collaboration with others as appropriate; exhibit a sense of ownership in decision-making activities.
  • Attend staff meetings and team conferences; participate in in-service education, staff meetings, and workshops for professional development.
  • Educate clinic staff about IBCCP services, providing in-services at staff meetings as needed.
  • Maintain orderliness and cleanliness of the work area.
  • Performs other duties as required or assigned.
Qualifications
  • Illinois licensure as a Registered Professional Nurse or Illinois licensure by reciprocity within six months.
  • Requires a Bachelor degree in Nursing, an Associate Degree in Nursing or graduation from a diploma School of Nursing.
  • With a Bachelor’s degree, one year of clinical case management or two years of work in a health-related field.
  • With an Associate’s degree, two or more years of clinical case management experience.
