St. Mary's Home Care & Hospice is seeking a full‑time Licensed Practical Nurse (LPN) for home‑care revisits in Athens, GA.

Our home‑care LPNs provide exceptional one‑to‑one care, using cutting‑edge technology for patient assessment, care planning, intervention and overall supervision of patient care and outcomes. Nurses follow physician orders and comply with the state Nurse Practice Act and our policies and procedures.

• Conduct client assessment and develop individualized care plans.
• Implement interventions and supervise patient care to achieve desired outcomes.
• Adhere to physician orders, state regulations, licensure and organizational policies.

• Graduate of an accredited or state‑approved school of practical nursing.
• Licensure as an LPN in the State of Georgia.
• Minimum one year of experience in an acute‑care setting; home‑care experience preferred.
• Phlebotomy and wound care experience required.
• Must have passed the National Association Practical Nurse Examination (NAPNES) or an equivalent medication exam before administering medications.
