Description: St. Mary s Home Care & Hospice is seeking a Full time Certified Home Health Aide/C.N.A. for hospice in the Athens, Georgia area. Trinity Health at Home; our agency provides compassionate, exceptional care offer a $2K sign-on bonus.

Responsibilities- Provide one-to-one, compassionate care to patients in their homes following physician orders and agency policies.
- Follow care plans developed by the RN and report client changes in condition to the RN.
- Assist clients with personal care activities (bathing, mouth and denture care, skin care, grooming, bathroom needs, transferring, ambulation, dressing).
- Assist with household tasks (meal planning and preparation, feeding, light housekeeping, changing linens, making beds, laundering clothes and bedding).
- Measure and record client vital signs as ordered (oral, rectal, axillary temperatures; pulse, respiration, blood pressure).
- Support rehabilitation by performing ROM and other simple procedures as assigned.
- Assist clients in self-administration of medication when assigned by an RN and documented in the clinical record; does not administer medications.
- High School diploma or GED.
- Completion of CNA 75-hour course.
- Completion of federally compliant training program and successful completion of a competency evaluation program as required by law/regulation.
- Documentation of one year of employment as an aide in an institution, home care, or hospice setting.
- Meets agency hiring criteria (e.g., professional references).
- Ability to function with minimal direct supervision and maintain professional relationships with clients and families.
- Commitment to ongoing skills development and adherence to agency policies and procedures.
- Ability to demonstrate organizational core competencies and ethics and to comply with the Compliance Program.
- Current driver s license and reliable transportation to and from the work site.
