Position Overview- Role:
Nurse Practitioner (NP) or Physician Assistant (PA) - Location:
Waterville and surrounding Central Maine communities - Schedule:
Part-time, 2–3 weekdays per week - option to become full-time - Care Model:
Primarily onsite with some telemedicine coverage
- Provide comprehensive geriatric medical care in assisted living, memory care, and long-term care facilities
- Manage chronic conditions, acute issues, and regulatory visits with a patient-centered approach
- Develop and oversee individualized care plans focused on function, safety, and quality of life
- Collaborate closely with nursing staff, facility leadership, and interdisciplinary care teams
- Participate in telemedicine visits to enhance continuity and access to care
- Use remote patient monitoring tools to support proactive and preventive care
- Complete timely, high-quality documentation in the EHR
- Active NP or PA license in the state of Maine
- Experience in geriatrics, long-term care, or post-acute care—or a strong interest in this specialty
- Comfort working independently while collaborating within a team-based model
- Strong clinical assessment, diagnostic, and decision-making skills
- Confidence using technology, EHRs, and telehealth tools
- Clear, compassionate communication with patients, families, and care teams
