Job Summary
We are seeking a dedicated Licensed Practical Nurse (LPN) for a 13-week travel assignment in a Long-Term Care facility in New York. The LPN will provide high-quality nursing care to residents with chronic conditions and geriatric care needs.

Assignment Details

• Contract Length: 13 Weeks
• Shift: 8 Hour Day Shifts
• Must have active NY license
• Schedule: To be determined by facility
• Setting: Long Term Care (LTC)
• Radius Rule: 100 Mile Radius Rule applies

Requirements

• Active New York LPN License – Required
• 1–3 years of nursing experience
• LTC experience preferred
• Candidates with Skilled Nursing Facility (SNF) experience will also be considered
• BLS Certification (AHA) – Required
• Experience with Point Click Care  eMAR system preferred

Patient Population
Common diagnoses include:

• Dementia
• Diabetes
• Chronic diseases
• Geriatric care needs

Responsibilities

• Provide direct patient care and monitor residents
• Administer medications and treatments
• Document care using Point Click Care  eMAR
• Work collaboratively with healthcare teams
• Ensure patient safety and adherence to care plans
